Key Responsibilities:

  • Provide clinical pharmacy services, including pharmacokinetic dosing, therapeutic interchange, and IV/PO conversions.
  • Review and verify physician medication orders and patient medical records.
  • Interact with healthcare professionals and providers to promote safe medication use and prevent inappropriate or misuse of medications.
  • Maintain and review patient medication profiles.

Requirements:

  • Doctor of Pharmacy Degree.
  • California Pharmacy license.
  • ASHP-accredited PGY2 oncology residency or at least a minimum of 1 year of pharmacy practice experience in oncology.
  • Recent clinical oncology experience (within 5 years).
  • Board Certification in Oncology (BCOP) preferred.
  • Previous infusion pharmacy experience preferred.
  • EPIC/Beacon experience preferred.
  • Exceptional communication, interpersonal, organizational, and follow-through skills.
  • Knowledge of and ability to perform age-specific and disease-specific pharmacokinetic calculations.
