Job Summary

We are seeking a skilled Physical Therapist Assistant to join our team at Uphams Corner Health Center. As a member of our interdisciplinary team, you will provide comprehensive care to older adults in their homes, promoting physical and functional mobility, and supporting their community re-entry.

Key Responsibilities
  • Conduct physical therapy assessments, reassessments, and interventions to older adults in their homes.
  • Promote and maximize physical and functional mobility, supervise rehabilitation aides as needed, and participate in clinical team meetings.
  • Provide participant, family, and staff education on home safety and community re-entry assessments.
  • Document patient progress, including treatment plans and progress notes.
  • Order adaptive equipment and perform other physical therapy-related duties as assigned.
Requirements
  • Valid Massachusetts Physical Therapist or Physical Therapist Assistant license.
  • CPR certification.
  • 2-3 years' clinical experience working with the chronically ill, cognitively impaired, or frail geriatric population.
  • Fluency in written and spoken English.
  • Ability to work within a multidisciplinary team setting.
  • Bi-lingual skills in Spanish, Haitian Creole, or Portuguese Creole are highly preferred but not required.
