Key Responsibilities- Collaborate with patients, families, social workers, physicians, and the interdisciplinary team to provide leadership and advocacy in care coordination.
- Manage clinical resources and transition planning for patients within an assigned caseload, identifying and eliminating barriers to care delivery.
- Work closely with the care team to develop and implement individualized care plans, ensuring patient-centered and evidence-based care.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices in care coordination.

Requirements- Completion of an accredited RN Nursing Program; BSN preferred.
- One year of RN experience required.
- Preferred experience in case management, discharge planning, or utilization review.
- Valid Maryland Nursing License required.
Salary range: $39.94 - $61.91 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
