RN Case Manager

In collaboration with patients, families, social workers, and physicians, the Case Manager provides leadership and advocacy in the coordination of patient-centered care across the continuum. This role involves managing clinical resources and transition planning for patients within an assigned caseload from pre-admission through post-discharge.
The ideal candidate will have completion of an accredited RN Nursing Program, one year of RN experience, and preferred experience in case management, discharge planning, or utilization review. A valid Maryland Nursing License is required.
Johns Hopkins Medicine offers a competitive salary range of $39.94 - $61.91 per hour, commensurate with equity and experience. We prioritize the health and well-being of every employee and strive to create a welcoming and inclusive environment.
