Job Description

As a Registered Nurse in our Nursing Care Coordination department, you will be responsible for providing direct nursing care to patients and coordinating with interdisciplinary teams to ensure seamless transitions of care. Your duties may include:

  • Implementing and monitoring patient care plans
  • Assessing and coordinating patient's discharge planning needs
  • Serving as a primary coordinator of all disciplines for well-coordinated patient care
Requirements

To be considered for this role, you must meet the following requirements:

  • Licensure / Certification / Registration: BLS Provider obtained within 1 month of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
  • Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
