About the Role
The VA Community Care Nurse Specialist plays a critical role in ensuring that Veterans receive seamless and coordinated care across different settings. As a skilled and empathetic healthcare professional, you will be responsible for managing complex client situations, developing personalized care plans, and collaborating with interdisciplinary teams to achieve optimal outcomes. With a deep understanding of the unique needs of Veterans, you will prioritize their care, advocating for their interests and well-being throughout the continuum of care.

Key responsibilities include providing expert clinical guidance, facilitating transitions between care settings, and promoting continuity of care. You will also educate patients and families about self-management strategies, disease prevention, and health promotion. With a focus on population-specific and age-specific care, you will deliver culturally sensitive care to meet the diverse needs of our Veteran population. By leveraging technology and data-driven insights, you will drive quality improvement initiatives and optimize resource utilization, ensuring that every Veteran receives the highest quality care possible.

This role offers a competitive salary of $ $94,459 - $124,037 per year, depending on experience.
