Job Title: LPN Medical Coordinator

At Resources For Human Development, we are seeking a skilled LPN Medical Coordinator to join our team. As a key member of our healthcare services team, you will play a vital role in ensuring the well-being of our program consumers.

Job Summary

The LPN Medical Coordinator will work closely with our program RN and site supervisors to coordinate healthcare services for our consumers. This role requires a strong commitment to our RHD values and a passion for delivering high-quality patient care.

Key Responsibilities
  • Maintain accurate and up-to-date nursing documentation and medication records.
  • Assist consumers in understanding their medications and ensure they are taken as prescribed.
  • Provide nursing care, including wound care, tracheostomy care, and diabetic counseling.
  • Assess consumers' health and emotional needs and communicate effectively with other healthcare professionals.
  • Coordinate medical appointments and ensure timely follow-up care.
  • Participate in multi-disciplinary team meetings and case conferences to discuss consumer care.
  • Develop and implement medical protocols to support consumer health and well-being.
Requirements
  • Current LPN license.
  • Ability to lift up to 50 pounds occasionally and 20 pounds frequently.
