Job Title: Care Partner

We are seeking a compassionate and dedicated Care Partner to join our team at Experience Senior Living. As a Care Partner, you will play a vital role in providing exceptional care and support to our residents, ensuring their physical, emotional, and social needs are met.

Responsibilities:
  • Assist residents with their personal care and activities of daily living, including bathing, dressing, and grooming.
  • Perform daily housekeeping tasks, such as cleaning and laundry, to maintain a clean and comfortable living environment.
  • Review and participate in 24 Hour Reports, shift change reports, and verbal reports to ensure seamless communication and care coordination.
  • Assist residents to and from scheduled life enrichment programs, promoting socialization and engagement.
  • Prioritize tasks to ensure optimum services to residents, adapting to changing needs and requests.
  • Communicate with the Director of Health & Wellness and other departments as needed to ensure resident care needs are met.
  • Respond to resident pendant/wearable calls in a timely manner, ensuring prompt assistance and support.
Requirements:
  • High School Diploma or equivalent.
  • Certified Nursing Assistant/Home Health Aid preferred, but not required.
  • License/Certificate for Medication Assistance (if applicable) as needed per state regulations preferred.
  • CPR certification/First Aid certification preferred.
  • Able to read and comprehend simple instructions and short correspondence.
  • Able to write simple correspondence and effectively present information in one-on-one and small group situations.
  • Able to apply common sense understanding to carry out detailed written or verbal instructions.
  • Ability to deal with difficult situations and communicate effectively with all levels of management, team members, residents, family members, vendors, referral sources, and outside contacts.
