About the Job

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Brooklyn Center for Rehabilitation and Healthcare. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our healthcare facility.

Job Summary

The LPN will be responsible for providing direct patient care, administering medications, and performing various clinical tasks. You will work closely with our interdisciplinary team to ensure that patients receive comprehensive and compassionate care.

Key Responsibilities
  • Collect and record patient information, including vital signs and medical history.
  • Administer medications and injections as prescribed by physicians.
  • Treat and dress wounds and bedsores as needed.
  • Supervise Certified Nursing Assistants (CNAs) and provide guidance and support as needed.
  • Assist patients with daily living activities, such as bathing and dressing.
  • Monitor patients' food and liquid intake and output.
Requirements
  • Valid LPN State license.
  • Must be in good standing with State Registry.
  • Ability to work as a team member and contribute to a positive and supportive work environment.
