  • Mental Health Support: Teach members about managing their mental health, creating a supportive environment for them to thrive.
  • Wellness Planning: Collaborate with members and the treatment team to create individualized wellness goals tied to their treatment plans, providing tools and resources to support goal attainment.
  • Group Facilitation: Facilitate group activities and individual sessions to promote improved health outcomes for members.
  • Coordination of Care: Work closely with the treatment team to engage members in appropriate services and resources, ensuring seamless coordination of care.
  • Documentation and Reporting: Complete all documentation and reporting requirements for outcomes and evaluation, ensuring accurate and timely reporting.
Requirements
  • Education: Minimum of a High School Diploma and/or associate degree with Wellness Certification; Bachelor's degree in a health-related field preferred.
  • Experience: Two (2) years of experience in a drug and alcohol setting, with a strong background in health coaching and mental health support.
  • Skills: Strong communication and interpersonal skills, with the ability to work effectively with diverse populations; proficiency in Microsoft Office.
  • Personal Qualities: High energy, strong work ethic, and ability to multi-task; self-motivated and able to work in a self-directed manner.
