We are seeking a highly skilled Clinical Pharmacist to join our team at UnitedHealth Group. In this role, you will have the opportunity to make a meaningful impact in the lives of our patients by providing patient-centered, clinically collaborative medication consultations.

This is an exciting opportunity for a motivated and compassionate individual to work with a talented team of healthcare professionals who share your passion for delivering high-quality care. You will have the opportunity to work closely with our interdisciplinary service delivery team, including Pharmacists, Pharmacy Technicians, Licensed Vocational Nurses, and Care Coordinators, to deliver comprehensive care and support our patients' healthcare needs.

Your primary responsibilities will include:
">• Designing and executing telephonic and digital engagement strategies to improve patient outcomes
">• Conducting in-depth assessments of patients' medical histories and current medication regimens to identify potential issues and opportunities for improvement
">• Developing personalized plans to address medication adherence barriers and optimize patient outcomes through improved medication management practices
">• Collaborating with healthcare providers to ensure seamless transitions of care and optimal patient outcomes

In addition to your primary responsibilities, you will also be expected to:
">• Analyze data and metrics to identify trends and areas for improvement in patient care
">• Develop and implement effective solutions to address gaps in care and improve patient outcomes
">• Stay up-to-date on the latest developments in clinical pharmacy practice and apply this knowledge to improve patient care
">What You'll Need:
">
To be successful in this role, you will need to have a strong foundation in clinical pharmacy practice, as well as experience working with Medicare STARs & HEDIS Measures. You should also possess excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team. Additionally, you must be able to lift up to 25 pounds, sit for extended periods, and use fine motor skills to operate office equipment and machinery.
