Job Summary

As a Hospice Nurse Specialist, you will coordinate patient care with a patient-centered approach and evaluate health responses. You will develop, implement, and revise care plans to achieve patient outcomes.

Key Responsibilities
  • Develop and implement individualized care plans for patients
  • Evaluate the quality and effectiveness of nursing practice
  • Support evidence-based practice changes through research
  • Participate in quality/performance improvement initiatives
  • Provide patient-centered care, respecting and incorporating patient and family values and goals
Requirements
  • BSN required
  • One year relevant work experience in Hospice or Home Health
  • Must be a registered nurse
  • Obtain Basic Life Support certification within 30 days of hire
