Position Overview:

As a vital member of our team, the Office and Patient Coordinator will focus on enhancing the quality of care for our older adult patients. This role is integral to ensuring a seamless operation within our healthcare facility, emphasizing the 3Cs: Care, Community, and Compliance.

Key Responsibilities:

  • Care: Collaborate with a dedicated interdisciplinary team, including a psychiatric physician, RN Program Director, and social worker, to deliver exceptional care to our older adult patients as per medical directives.
  • Community: Foster a culture of kindness and accountability, working alongside the program team to achieve outstanding care, compliance, and community objectives.
  • Compliance: Ensure adherence to all regulations governing clinical, billing, and operational practices in partnership with the program team.

  • Proficient time management
  • Team-oriented mindset, capable of collaborating within a small interdisciplinary group
Qualifications:
  • CNA, LPN, or MA credentials are preferred
  • Valid driver's license and ability to transport patients are required
  • Experience in a medical office setting is preferred
  • Genuine passion for working with older adults is preferred
  • Front desk or administrative experience is preferred
Work Environment:
