We are a Federally Qualified Health Center (FQHC) located in Manhattan, committed to delivering high-quality, comprehensive, and affordable primary and specialty care to diverse and underserved communities.
Position Type:
Full-Time / Direct-Hire
Work Schedule:
Monday to Friday: 9 AM - 5 PM with potential late shifts on Tuesday or Thursday until 7 PM.
Key Responsibilities:
- Provide medical services typical of a family practitioner, including examination, diagnosis, and treatment of various health conditions.
- Manage a patient caseload that meets quality care and productivity standards.
- Maintain ongoing communication with healthcare providers regarding patient health needs.
- Complete necessary documentation, including progress notes and forms required by regulatory agencies.
- Participate in medical staff meetings and continuous education activities.
- Supervise and mentor paramedical staff in patient care activities.
- Deliver care to patients with both acute and chronic medical issues, ensuring appropriate treatment and follow-up.
- Develop and implement individualized care plans for patients, ensuring continuity of care.
- Engage in quality improvement initiatives and peer review sessions.
- Adhere to infection control standards and report any unusual occurrences to supervisors.
Qualifications:
- 1-2 years of experience in an Ambulatory Care Setting and Community Health Care.
- Family Nurse Practitioner or Physician Assistant Certification.
- Current New York State License and Board Certification.
Competencies:
- Exhibit a professional and respectful demeanor in patient interactions.
- Demonstrate the necessary skills to provide age-appropriate care.
- Collaborate effectively with team members to enhance patient outcomes.
Salary: $117k - $131k
