Job Summary

We are seeking a highly skilled Nurse Practitioner to join our team at NYC Health + Hospitals. As a Primary Care Specialist, you will be responsible for providing high-quality, patient-centered care to our diverse patient population.

Key Responsibilities
  • Provide comprehensive medical and nursing care to patients, including preventive and supportive health services.
  • Manage health problems and coordinate care for various patient populations.
  • Identify and diagnose illnesses and physical conditions, and implement therapeutic and corrective measures.
  • Develop and maintain partnerships with community-based organizations and make referrals to consulting physicians and/or other healthcare professionals and community agencies, as necessary.
  • Evaluate and interpret patient data, and plan and implement appropriate nursing interventions.
  • Consult with physicians and other healthcare practitioners in the evaluation of health screening findings to make prospective decisions about treatment.
  • Formulate differential diagnosis and the plan of care based on medical history, physical exams, nursing assessment, and other pertinent information.
  • Develop, implement, and evaluate patient plans of care, determining effectiveness and modifying as needed to achieve medical and health goals.
  • Order, conduct, and interpret diagnostic and laboratory tests, and identify and prescribe appropriate pharmacological and non-pharmacological interventions and therapies.
  • Manage patient care as outlined in practice protocols, and collaborate with practice agreements and authorized by Medical Board.
  • Establish medical diagnosis as well as nursing diagnosis as a Certified Nurse Practitioner of New York in accordance with Article 139, Section.
  • Record and document data necessary to maintain and coordinate healthcare.
  • Be a clinical/educational resource person for nursing personnel.
Professional Practice
  • Participate in the development, implementation, and evaluation of interdisciplinary Performance Improvement (PI) activities for the specialty.
  • Seek constructive feedback and take action to achieve goals identified in performance appraisal.
  • Provide clinical and didactic instruction to professional staff, students, and community groups, and conduct inservice educational programs.
  • Initiate and participate in studies and research related to nursing practitioner services, healthcare, and allied services.
  • Participate in hospital/nurse practitioner committees.
  • Provide clinical supervision to Nurse Practitioners and students.
  • Participate with all staff to achieve zero defects goal for the Hospital.
Governance
  • Consider factors related to safety, effectiveness, and cost in the planning and provision of patient care.
  • Act as a liaison between nursing and professional staff assigned to the specialty.
  • Utilize the Just Culture Algorithm to manage risk and prevent adverse outcomes.
  • Participate in programs to improve communication, safety, and interdisciplinary teamwork.
Self-Development
  • Evaluate own practice in relation to professional standards and regulatory requirements.
  • Maintain clinical competency by keeping abreast of current trends in healthcare, legislation, regulatory requirements, and nursing practice.
Patient Satisfaction
  • Anticipate patient needs and, within scope and authority, take initiative to meet those needs, keeping patients and families informed about actions taken.
  • Provide consistent and timely follow-through, ensuring that all customer requests are addressed in a prompt and courteous manner.
  • Make every attempt to defuse potentially volatile situations.
  • Resolve differences in a spirit of cooperation, creating solutions that benefit all parties.
  • Able to maintain composure, efficiency, and positive customer service-oriented manner during periods of peak workload.
  • Participate in the Customer Satisfaction initiatives.
  • Adhere to the ICARE standards of Customer Service.
Minimum Qualifications
  • A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED).
  • Current certificate as a Nurse Practitioner issued by the NYSED.
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
