We are seeking a skilled Care Coordinator to join our team at Martin Luther King Jr. Community Hospital. As a Care Coordinator, you will play a critical role in ensuring the seamless transition of patients through the healthcare system.
Key Responsibilities- Act as a patient advocate and health systems navigator, coordinating continuity of care with external healthcare organizations and facilities.
- Coordinate patient care plans with patients and families/caregivers, ensuring effective communication and collaboration.
- Monitor and report care barriers and challenges to appropriate care managers.
- Conduct comprehensive, preventive screenings for patients and assist support staff in daily patient interactions.
- Support patient self-management of disease processes and promote behavioral modifications.
- Facilitate patient medication management based on standing orders and protocols.
- Participate in data collection, health outcomes reporting, clinical audits, and pragmatic evaluation.
- High School Diploma/GED equivalent required; college degree or vocational training in healthcare field preferred.
- 1-2 years healthcare facility experience and familiarity preferred.
- Strong organizational and communication skills, with ability to work effectively in a fast-paced environment.
- Current Basic Life Support (BCLS) for Health Care Providers certification from the American Heart Association.
