LHC Group is seeking a skilled Registered Nurse (RN) to join our Home Health team in Houghton, Michigan.
About the Role:We are looking for a compassionate and dedicated RN to provide high-quality patient care in the comfort of their own homes. As a Registered Nurse - Home Health, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and providing education and support to patients and their families.
Responsibilities:- Conduct comprehensive assessments of patients' physical and emotional needs
- Develop and implement individualized care plans to meet patients' needs
- Provide education and support to patients and their families
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Document patient progress and outcomes
- Current RN licensure in the state of Michigan
- One year of clinical experience
- Current CPR certification
- Valid driver's license and access to a dependable vehicle
