Goshen Health is seeking a skilled Care Coordinator to join our team. As a key member of our healthcare team, the Care Coordinator will be responsible for ensuring the exceptional experience of care for our patients and their families.
Key Responsibilities:
- Psychosocial Assessment and Clinical Intervention: Conduct thorough psychosocial assessments to identify complex care needs and develop effective clinical interventions to address these needs.
- Short-Term Therapeutic Intervention and Crisis Management: Provide short-term therapeutic interventions and crisis management to patients and their families, ensuring timely and effective resolution of issues.
- Care Coordination: Coordinate care across the continuum, collaborating with physicians, continuity of care planners, and other interdisciplinary team members to ensure seamless transitions and optimal patient outcomes.
- Resource Development and Utilization: Identify and develop resources to support patients and their families, reducing unplanned procedures, visits, and admissions.
- Transition Planning and Patient Engagement: Engage patients and their families in transition planning, ensuring alignment with goals, department priorities, and system quality initiatives.
- Communication and Collaboration: Communicate effectively with all members of the healthcare team, facilitating the episode of care and ensuring timely and safe delivery of services.
Requirements:
- Licensure: Permanent LSW licensure is required. LCSW licensure is preferred.
- Education: A Master's Degree in Social Work is required.
- Experience: Relevant healthcare, mental health, or community-based experience is required.
- Skills: Strong clinical assessment, planning, and goal development skills are required. Ability to communicate and resolve conflict with diverse stakeholders is essential.
