Job Description

A Nurse Practitioner in Long-Term Care plays a critical role in providing high-quality patient care to our residents. This individual will work closely with our interdisciplinary team to develop and implement personalized care plans that cater to the unique needs of each resident.

Responsibilities include:

• Conducting thorough assessments of patients' physical and emotional needs
• Developing and implementing comprehensive care plans that address these needs
• Collaborating with other healthcare professionals to ensure seamless care coordination
• Maintaining accurate and detailed records of patient interactions and progress

Requirements

To be successful in this role, you will need:

• A valid RN state license and CPR certification
