Hospice RN Case Manager Job Summary

We are seeking a compassionate and skilled Hospice RN Case Manager to join our team at Interim HealthCare Staffing. As a Hospice RN Case Manager, you will play a vital role in providing exceptional care to patients and families in Charleston, Berkeley, and Dorchester Counties.

Key Responsibilities:
  • Maintain a specified case management load, ensuring timely and effective care delivery.
  • Conduct comprehensive assessments of patients and families to determine hospice needs, including physical and psychosocial evaluations.
  • Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families.
  • Coordinate care among IDG members and external healthcare providers to ensure seamless delivery of services.
  • Evaluate and coordinate the need for services, including inpatient, respite, and continuous care.
  • Supervise and direct LPN/LVN, Hospice Aide, and Homemaker services as needed.
Requirements:
  • Graduate of an accredited school of nursing with a current RN license in South Carolina.
  • 12 months of accumulated experience as a registered nurse within the last five years or evidence of current clinical knowledge.
  • One year of hospice care experience and one year of precepting or training experience preferred.
  • CPR certification and valid driver's license with auto liability insurance.
