Hospice RN Case Manager Job Summary

The Registered Nurse (RN) plays a vital role in planning, organizing, and directing hospice care. This position requires a skilled nurse to supervise and coordinate care for patients with terminal illnesses, diseases, and compromised health states.

Key Responsibilities:
  • Provide comprehensive skilled nursing services to patients in their homes or hospice inpatient setting.
  • Supervise and coordinate care with other healthcare professionals and community agencies.
  • Administer medications, assess symptoms, and provide education and teaching to patients and their families.
  • Work independently with minimal supervision, prioritizing workload and managing time effectively.
Requirements:
  • Graduate of an accredited school of nursing.
  • One year of professional nursing experience in an acute care, home care setting, or hospice inpatient unit preferred.
  • Current licensure as a Registered Nurse in the state of South Carolina or a compact/multi-state license.
  • CPR certification.
Work Environment:

This position requires frequent standing, walking, bending, and driving. The nurse will work in various settings, including patients' homes and community agencies. The work environment may involve exposure to communicable diseases, body fluids, and other hazardous conditions.
