About the Role
As a member of our rehabilitation team, you'll work closely with patients, families, and healthcare professionals to develop and implement personalized plans of care. Your expertise will be invaluable in helping patients overcome mobility challenges and achieve optimal outcomes.
Key Responsibilities
* Provide home-based physical therapy to patients with mobility, strength, and functionality challenges
* Work collaboratively with a multidisciplinary team to develop and implement patient care plans
* Conduct assessments, document progress, and modify care plans as needed
* Assist patients with exercises and hands-on therapy to manage pain and increase mobility
* Educate patients and families on therapy goals, exercises, and self-care
Requirements
* Active PTA license in Maine
* Minimum of 1 year of PTA experience, ideally in home healthcare
* CPR certification
* Up-to-date on all State of Maine Required vaccinations
* Knowledge of state and federal home health regulations
* Good clinical judgment, strong interpersonal skills, dexterity, and compassion
