Clinical Nurse Practitioner or Physician Assistant

Matrix Medical Network is seeking a skilled Nurse Practitioner or Physician Assistant to join our team in a PRN/Part Time capacity. As a Home Risk Assessment provider, you will play a vital role in reviewing and observing patients' current health, medical history, medication adherence, social environment, and other risks.
Key Responsibilities:
- Conduct comprehensive health assessments, medical history, diagnosis, and treatment
- Collaborate with physicians, case managers, social workers, and other healthcare professionals
- Provide education and support to patients and their families
- Work in a variety of settings, including home visits, skilled facilities, and telehealth
Requirements:
- Master's degree in Nursing or Physician Assistant studies
- Current RN and NP or PA licensure in the state of practice
- Board certification in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
