Job Title: Hospice RN Case Manager

At Interim HealthCare - North Charleston, SC, we are seeking a compassionate and skilled Hospice RN Case Manager to join our team. As a Hospice RN Case Manager, you will play a vital role in providing exceptional care to patients and their families in Charleston, Berkeley, and Dorchester Counties.

Job Summary:

The Hospice RN Case Manager will be responsible for maintaining a specified case management load, completing initial and ongoing assessments of patients' physical and psychosocial status, and developing and implementing individualized plans of care in collaboration with the Interdisciplinary Group (IDG) and the attending physician.

Key Responsibilities:
  • Maintain a specified case management load, ensuring timely and effective care coordination.
  • Conduct initial and ongoing assessments of patients' physical and psychosocial status, identifying needs and developing individualized plans of care.
  • Collaborate with the IDG and attending physician to develop and implement plans of care, ensuring continuity of care and achievement of patient goals.
  • Coordinate care among IDG members and other healthcare providers, ensuring seamless delivery of services and effective communication.
  • Evaluate and coordinate the need for services, including inpatient, respite, and continuous care, as indicated by patients' status and needs.
  • Supervise and direct services provided by LPN/LVN, Hospice Aide, and Homemaker, ensuring high-quality care and adherence to IDG plans.
  • Facilitate consideration of alternative levels of care when indicated by patients' status and needs.
Requirements:
  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina, with an active and good-standing license.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
  • One (1) year of hospice care experience, with preference for precepting or training experience.
  • CPR certification.
  • Able to provide proof of valid driver's license and auto liability insurance.
