Responsibilities:
  • Provide comprehensive healthcare services, including assessment, diagnosis, and treatment planning.
  • Collaborate with PCPs and interdisciplinary teams to ensure seamless care coordination.
  • Develop and implement effective treatment plans that meet individual patient needs.
  • Provide ongoing education and support to patients and their families.
  • Maintain accurate and compliant documentation within our EMR system.

Requirements: To be successful in this role, you must have a Master's degree in Nursing or a related field, national certification as an NP, and an active state license. At least 2 years of clinical experience as a Nurse or Nurse Practitioner in geriatric, adult, or family practice settings is required. Strong communication and interpersonal skills are essential for success in this role. Additionally, you must be passionate about delivering quality healthcare to senior populations and committed to ongoing professional development.

Salary Range: $130,000 - $160,000 per year.
