About the Role

CenterWell Home Health is seeking a skilled Home Health Physical Therapist Assistant to join our team. As a key member of our care team, you will work closely with patients in their homes, providing personalized physical therapy services to help them recover from injury or illness.

Key Responsibilities
  • Administer physical therapy treatment plans as developed by the Physical Therapist
  • Provide active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients
  • Accurately document daily, weekly, or monthly records of clients' progress and submit clinical paperwork on time
  • Attend educational meetings and contribute to departmental in-services
  • Notify physical therapist or Executive Director of any equipment failures, lack of supplies, staff or client changes, and other departmental concerns
  • Schedule clients and perform varied office tasks
  • Apply concepts of infection control and universal precautions in performing patient care activities
  • Accept clinical assignments consistent with education and competence to care for patients
  • Promote compliance with fiscal intermediaries and/or other third-party payors through education, coaching, and assistance

Requirements
  • Associate degree in Physical Therapy from an American Physical Therapy Association
  • Current state license to practice Physical Therapy
  • Valid driver's license, auto insurance, and reliable transportation
  • Proof of current CPR certification
  • Minimum two years experience as a Physical Therapy Assistant
  • Comprehensive knowledge of geriatric population
  • Self-directed, enthusiastic, and accepts constructive feedback
  • Good verbal and written communication and organization skills
  • Professional image/attitude for patients, families, clients, coworkers, and others
  • Maintains clinical competency in Physical Therapy practice and theory
  • Able to work a flexible schedule and travel locally
  • Maintains confidentiality
  • Must read, write, and speak fluent English
  • Must have good and regular attendance
