">We are seeking a skilled Licensed Practical Nurse (LPN) to join our hospice care team. As an LPN, you will provide palliative nursing care to patients in their homes or care facilities, working closely with our interdisciplinary team to ensure comprehensive care.

- Deliver high-quality palliative nursing care to patients, including pain management and symptom management.">
- Collaborate with patients, caregivers, physicians, and the interdisciplinary team to ensure comprehensive care.">
- Develop and implement individualized care plans to meet patients' physical, mental, social, and emotional needs.">
- Maintain accurate records and documentation, adhering to agency policies and procedures.">
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.">
- Stay up-to-date with the latest medical research, technologies, and best practices in palliative care.">
- Participate in Quality Improvement initiatives to enhance patient outcomes and satisfaction.">
Requirements

- A current, active, and unrestricted LPN licensure in the state of practice.">
- Strong communication, interpersonal, and problem-solving skills.">
- Ability to work independently and collaboratively as part of a multidisciplinary team.">
- Flexibility to work a variety of shifts, including evenings, weekends, and holidays.">

Compensation
">The estimated annual salary for this position is $75,000 - $95,000, based on experience and qualifications. Benefits include comprehensive health insurance, paid time off, and opportunities for professional growth and development.
