Job Summary

Acute Home Healthcare is seeking a skilled Speech Language Pathologist to join our team of healthcare professionals. As a Speech Language Pathologist, you will play a vital role in improving the communication skills of our patients and helping them return to optimal productivity.

Key Responsibilities
  • Administer speech therapy to patients in their place of residence, as ordered by physicians and in accordance with plan of care.
  • Conduct initial evaluations, including plans of treatment and goals, and submit to physicians for approval.
  • Provide full-range Speech-Language Pathology services, including directing and supervising personnel as assigned.
  • Participate in patient care planning and consult with physicians regarding changes in treatment.
  • Write reports to physicians on patient progress and instruct patients and family members in home programs.
  • Assist physicians in evaluating patient level of function and help develop plans of care.
  • Prepare clinical and progress notes, and advise and consult with family and other Agency personnel.
  • Participate in in-services programs and complete and submit assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with Agency-defined time frames.
Requirements
  • Certificate of Clinical Competency in Speech Pathology or audiology granted by the American Speech-Language Hearing Association.
  • Currently licensed in the state(s) in which practicing.
  • Two (2) years experience, preferred.
Working Environment

Works indoors in patient homes and travels to/from patient homes.

Supervised by Director of Clinical Services/Nursing Supervisor.

High.

Lifting Requirements

Ability to participate in physical activity, moderate lifting, and extensive bending, lifting, and standing on a regular basis.
