Job Summary

Under the general supervision of the Clinical Manager, coordinates the caseloads of peritoneal dialysis and home hemodialysis patients with ongoing care. Provides individualized home dialysis training to patients and families, ensuring they can perform treatments safely and in accordance with hospital and departmental policies.

Responsibilities
  1. Provides ongoing follow-up to patients and families in the home environment through home visits and/or telephone communications.
  2. Utilizes primary nursing and follows patients with monthly clinic visits.
  3. Communicates relevant information regarding patient needs or circumstances to appropriate personnel and/or healthcare team.
  4. Recognizes potential problem situations and acts accordingly.
  5. Takes on-call to cover patients after hours and is able to safely triage and assess problems.
  6. Administers intravenous, intraperitoneal, intramuscular, and subcutaneous medications safely following proper policies and procedures.
  7. Draws lab work on patients using proper procedures.
  8. Individualizes training for patients/caregivers using either home hemodialysis machine or CAPD.
  9. Assesses patient independently for complications of home dialysis and follows appropriate protocols for treatment.
  10. Manages dialysis prescriptions and provides adequate dialysis to patients.
  11. Makes home visits to assess the home environment to ensure patient is performing home dialysis in a safe and clean environment.
  12. Develops individualized plan of care establishing intervention and patient outcomes in collaboration with physician, other healthcare team members, patient, and significant other.
  13. Trains patients in ordering supplies and managing their inventory at home.
  14. Documents all training, home visits, care plans, assessments, and ongoing documentation in progress notes.
  15. Provides in-service to hospital and nursing home staff on home dialysis procedures and principles to ensure patient is receiving adequate and proper care in these facilities.
  16. Provides education on different modalities of dialysis to pre-dialysis patients and their families.
  17. Serves as a role model/preceptor to new graduate nurses and less experienced personnel.
  18. Assumes charge nurse responsibility for the home care dialysis area as delegated.
Requirements

Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.

Baccalaureate in Nursing preferred. Certified Nephrology Nursing Certification preferred. Must have current licensure to practice as a Registered Nurse in the State of Rhode Island.

Demonstrates knowledge and skills necessary to provide care to patients throughout the life span with consideration of aging processes, human development stages, and cultural patterns in each step of the care process.

Must have good driving record, meet, and maintain the appropriate and valid driver's license and insurance to operate a personal motor vehicle as required by the Federal Motor Carrier Safety Regulations.

Experience

Must have a minimum of 12 months of nursing experience and an additional 3 months of specialized dialysis experience providing clinical nursing care to dialysis patients in either a chronic or acute setting.

Experience must include dialysis focus of renal and hematological systems such as managing fluid and electrolyte balances, hemodynamics such as anemia management, iron management, bone disease management, and being able to differentiate renal lab values from normal lab values.

Work Environment and Physical Requirements

Visual and aural ability to monitor patient status. Physical ability to assist in lifting and positioning patients. Exposed to patient care environment. Physical ability for extensive walking and standing; lifting and positioning patients. Exposed to seasonal variations in weather including temperatures extremes and road conditions when traveling.
