CAREvity, LLC is seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Care Specialist. As an LPN, you will play a vital role in providing high-quality, patient-centered care to our clients in the comfort of their own homes.
Key Responsibilities:- Provide skilled nursing care to clients in their homes, including medication management, wound care, and vital sign monitoring.
- Develop and implement individualized care plans in collaboration with clients, families, and healthcare providers.
- Conduct thorough assessments of clients' physical and emotional needs, identifying areas for improvement and developing strategies for care.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and coordination.
- Maintain accurate and detailed records of client care, including progress notes, medication lists, and treatment plans.
- Communicate effectively with clients, families, and healthcare providers to ensure informed decision-making and optimal care outcomes.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Current LPN licensure in the state of practice.
- Minimum 3 years of experience in home care or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong problem-solving and critical thinking skills.
- Proficiency in electronic medical records and other healthcare software.
- Valid driver's license and reliable transportation.
