Job Title: Licensed Practical Nurse

CAREvity, LLC is seeking active Licensed Practical Nurse (LPN) students to join our team as a home health nurse. As a LPN, you will provide high-quality care to our clients in a private setting.

Responsibilities:
  • Maintain communication with clients' physicians and other healthcare providers.
  • Document clients' progress notes and clinical notes.
  • Ensure continuity of care in case of client transfer.
  • Coordinate care services between CAREvity, LLC, and other healthcare providers.
  • Preserve clients' confidentiality and agency matters.
  • Submit accurate time records and reports.
  • Respond to emergencies and participate in weekend and holiday shifts.
  • Participate in special projects, including surveys and studies.
  • Supervise direct care workers and ensure adherence to dress code and attendance policies.
  • Report suspected or witnessed cases of abuse and serious incidents.
  • Participate in policy formation and revision.
  • Assist the Administrator in making decisions related to care provision and services.
  • Comply with and ensure compliance with agency policies and procedures.
  • Protect CAREvity, LLC's resources, supplies, equipment, and property.
Qualifications and Education Requirements:
  • First Aid and CPR certifications.
  • 5 years of work experience in a facility, agency, or home care setting.
  • Excellent organizational and supervisory skills.
  • Ability to think and act quickly in emergency situations.
  • Kind, compassionate, and non-judgmental attitude.
  • Computer literacy and excellent communication skills.
  • Team player with ability to work with clients and colleagues.
  • Ability to work under stressful conditions with or without direct supervision.
  • Understanding of industry rules and regulations.
  • Willingness to comply with state and federal laws and agency policies.
  • Ability to demonstrate a full understanding of Universal infection control and OSHA procedures.
  • Physical ability to stand for long hours and lift or support clients with mobility issues.
  • Excellent visual and auditory senses.
  • Pass a background screen.
  • Valid driver's license and automobile insurance.
  • Ability to provide evidence of adequate health status to perform duties.
Physical Demands:

Our LPNs often spend long hours on their feet and require stamina. They may also help lift patients and support those who need physical assistance to walk, rise from the bed or toilet seat, etc.

  • Standing and sitting for more than 20 minutes.
  • Pushing, pulling, lifting, crawling, crouching, kneeling, climbing, and stooping.
  • Touching, feeling, holding, and handling.
  • Talking and listening.
  • Walking and running.
  • Tasting and smelling.
  • Close vision, peripheral/indirect vision, distant vision, and depth perception.
