Job Overview

This role offers an exciting opportunity for a skilled Licensed Practical Nurse (LPN) to join our team and provide one-on-one care to patients in their homes. As a key member of our multidisciplinary team, you will work closely with healthcare providers to deliver comprehensive and patient-centered care.

Key Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, catheter care, medication administration, symptom management, and other related tasks.
  • Conduct thorough observations, assessments, and evaluations to identify patient needs and develop individualized care plans.
  • Collaborate with healthcare providers and other caregivers to ensure seamless care transitions and optimal patient outcomes.
Requirements
  • Current WA Licensed Practical Nurse (LPN) licensure required.
  • Prior home health or hospice experience preferred but not necessary.
  • Ability to work independently and demonstrate critical thinking skills.
  • Current BLS certification from AHA.
Salary and Benefits
