About the Position

As a Nurse Practitioner - PRN/Part Time Home Risk Assessment Specialist with Matrix Medical Network, you will play a critical role in improving the quality of care for at-risk individuals. You will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and providing personalized care recommendations.

Responsibilities
  • Conduct thorough health assessments in patients' homes, including reviewing medical history, medication adherence, and social environment
  • Develop and implement personalized care plans to address identified health risks and improve patient outcomes
  • Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy behaviors and disease management
  • Document all assessments and care plans in accordance with Matrix Medical Network's guidelines and protocols
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice, including prescription authority
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent verbal and written communication skills
