Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time provider. As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to work with a diverse range of patients and provide comprehensive health assessments in the comfort of their own homes.

• Conduct comprehensive health assessments of patients in their homes, including medical history, medication adherence, social environment, and other risks.
• Develop and implement individualized care plans to address the unique needs of each patient.
• Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless care coordination.
• Provide education and support to patients and their families to promote healthy behaviors and improve health outcomes.

• Master's degree in Nursing or related field.
• Current RN and NP licensure in state of practice, including prescription authority.
• Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
• Minimum of 1 year experience as a Nurse Practitioner.
• Strong computer skills and familiarity with electronic health records.
