We are seeking a skilled LPN to join our team as a Float Nurse, providing quality care to patients in their end-of-life journey. As a member of our team, you will have the opportunity to make a meaningful impact on the lives of our patients and their families.
Key Responsibilities- Provide nursing care to patients in a manner that meets their needs and preferences.
- Work collaboratively with the RN Care Manager to ensure high-quality patient care.
- Travel to various locations to provide care, including homes and facilities.
- Participate in ongoing education and training to enhance your skills and knowledge.
- Registered LPN with the State of Ohio.
- One year of previous nurse experience.
- CPR certification.
- Certification in Hospice & Palliative Medicine (CHLPN) preferred.
- Valid Driver's License with a safe driving record.
