Job Summary

PATH Integrated Health is seeking a highly skilled Nurse Practitioner to join our team in providing exceptional mental health care services to our clients. As a key member of our outpatient care team, you will be responsible for assessing, diagnosing, and treating individuals and families, providing client education and referrals, and collaborating with our medical director and other healthcare professionals.

Key Responsibilities:
  • Assess and diagnose clients, developing treatment plans and implementing interventions as needed
  • Provide education and support to clients and their families, promoting healthy coping mechanisms and self-care strategies
  • Collaborate with our medical director and other healthcare professionals to ensure comprehensive care and coordination
  • Order and interpret laboratory screenings, reviewing documentation of prior care and consulting with prior care providers as necessary
  • Prescribe medications within the scope of your practice, maintaining follow-up care with clients and families
  • Exemplify the mission and vision of PATH Integrated Health, promoting a culture of compassion and excellence in mental health care
Qualifications:
  • Master of Science in Nursing (MSN) degree from an accredited university
  • Board of Nursing licensure - Psychiatric Mental Health Nurse Practitioner (Preferred) and Active DEA required
  • Minimum of 3 years nursing experience, with a minimum of 1 year in a behavioral health environment
  • Thorough knowledge of billing and coding guidelines, as well as electronic health records and telehealth services
