Key Responsibilities- Resident Assessment and Care Planning: Conduct and coordinate the development and completion of resident assessments (MDS) by current rules, regulations, and guidelines.
- Accuracy and Timeliness: Ensure accurate and timely completion of the MDS process.
- Interdisciplinary Collaboration: Collaborate with the interdisciplinary team to develop individualized resident care plans.
- Progress Monitoring: Monitor and document resident progress and responses to care plans.
- Quality Assurance: Participate in the Quality Assurance and Performance Improvement (QAPI) process.
- Communication: Communicate with residents, families, and other healthcare providers to coordinate optimal care delivery.
- Regulatory Compliance: Stay updated on MDS regulations and guidelines to ensure compliance.
- Competitive Pay: Up to $84,000 annually.
