OVERVIEW: PATH Integrated Health is a progressive leader in the field of mental health services, seeking exceptional professionals to enhance our dedicated team. As we expand our operations, we are looking for a Nurse Practitioner to join our collaborative care team, working alongside nurses, nurse practitioners, case managers, and therapists within our integrated agency. This is a unique opportunity to make a significant impact in the realm of mental health.
PRIMARY DUTIES:
- Conduct assessments, diagnoses, and treatment plans for individuals and families.
- Deliver client education and facilitate appropriate referrals.
- Engage in collaborative care conferences with the Medical Director and other healthcare providers.
- Order and interpret laboratory tests for clients.
- Review previous care documentation and consult with former care providers.
- Prescribe medications in accordance with your practice scope.
- Ensure ongoing follow-up care for clients and their families.
- Uphold the mission and vision of PATH Integrated Health.
REQUIREMENTS:
Education: Master of Science in Nursing (MSN) from an accredited institution.
Licensure: Active Psychiatric Mental Health Nurse Practitioner license and DEA registration required.
Experience: Minimum of 3 years in nursing, with at least 1 year in a behavioral health setting. Strong understanding of billing and coding practices.
PHYSICAL DEMANDS: Ability to work in high-pressure situations and irregular hours. Prolonged periods of sitting, computer use, and typing are required.
COMMUNICATION SKILLS: Excellent verbal and written communication skills are essential for professional interactions via phone and in writing. Strong grammar and writing abilities are necessary for effective communication with clients and colleagues.
TECHNICAL SKILLS: Proficiency in Electronic Health Record (EHR) systems, telehealth services, and other relevant software is required to manage client information efficiently. Fine motor skills and dexterity are necessary for various tasks.
BACKGROUND CHECK: Candidates must successfully pass all federal and state background checks, and their educational and professional history will be verified in accordance with company policies.
