Job Summary

MercyFirst is seeking a highly skilled and compassionate Therapeutic Case Manager to join our Refugee Services Long Term Foster Care team. As a key member of our treatment team, you will provide coordinated services between our children and their sponsors, ensuring their safe release from care and positive outlook for their future.

Responsibilities
  • Conduct regular casework sessions with each client in accordance with Federal, State, and Local mandates.
  • Maintain regular contact with birth parents, legal guardians, and/or resource persons in accordance with mandates.
  • Document all contacts in appropriate computer systems within established timeframes.
  • Prepare all required documentation on each case, including progress notes, psychosocial reports, and permanency hearing notes.
  • Attend legal proceedings, such as family court hearings.
  • Participate in staff training and supervision meetings.
  • Arrange, monitor, and document home visits, family/sibling visits.
  • Respond to emergencies as needed and/or directed.
  • Attend treatment team meetings with clinical and case manager supervisors, assigned clinician, program manager, etc.
Requirements
  • Bachelor's degree in social work or related field.
  • 1 year of related experience working with children/youth and families.
  • Bilingual (Spanish) strongly preferred.
  • Valid driver's license strongly preferred.
  • Must successfully complete all required training.
