Job Summary

We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our team as a Home Health LVN/LPN. As a Home Health LVN/LPN, you will provide nursing visits to patients in their homes, delivering personalized care and support to help them recover and maintain their independence.

Responsibilities
  • Provide nursing visits to patients in their homes, following treatment plans prescribed by physicians
  • Assess patients' physical and emotional needs, and develop individualized care plans to meet those needs
  • Administer medications, perform wound care, and provide other nursing interventions as needed
  • Document patient care and progress in medical records
  • Collaborate with other healthcare professionals to ensure seamless care coordination
Requirements
  • Graduate of an accredited practical or vocational nursing program
  • Currently licensed as a Practical Nurse or Vocational Nurse in the state of agency operation
  • Valid CPR certification
  • Ability to travel to all business locations
