Job Title: Case Manager - Substance Use Disorder Specialist

Job Summary:

Prevention Links Inc is seeking a highly skilled Case Manager - Substance Use Disorder Specialist to join our team. The successful candidate will work in partnership with peer recovery specialists to provide workforce support participants with assistance and support for issues related to substance use disorder.

Key Responsibilities:

  • Provide comprehensive case management assessments that address life domains such as housing, finances, transportation, legal services, vocational, employment, health care, and family strengths/needs.
  • Work collaboratively with peer recovery specialists to identify participant needs and facilitate responsive referrals to services.
  • Link to appropriate care and resources in the community, including resources that address specialized needs such as HIV/AIDS, mental health disorders, chronic and acute health problems, and problems stemming from involvement with the criminal justice system.
  • Promote the individual's recovery to their fullest potential by facilitating assessment of skills, including literacy, educational opportunities, job readiness skills, vocational training, employment placement and retention, and career development.
  • Refer clients and help them access substance use disorder treatment if needed.
  • Coordinate needed interventions and services provided by multiple agencies.
  • Provide case management services to ensure that individuals are both informed about and able to access the full range of medical and treatment opportunities or other support services, such as legal, financial, educational, vocational, or employment counseling, that may be available to them at the time the individual chooses to use them.
  • Use engagement skills to motivate and encourage individuals to voluntarily access services that would assist them in maintaining a healthy lifestyle.
  • Advocate on behalf of the individual.

Requirements:

  • Bachelor's level degree in health, psychology, counseling, social work, education, or other behavioral health professions.
  • At least three (3) years' experience working with high need individuals involved with substance use disorder.
  • Bilingual preferred.
  • Excellent oral and written communication skills.
  • Sensitive, flexible, and responsive to gender, race, ethnicity, socio-economic status, religion, age, sexual orientation, or any other special needs as reflected in the ability to communicate with staff and/or clients.
  • Proficient with Microsoft Word, Excel, PowerPoint, Outlook, and possess basic computer skills.
  • Authorized to work in the U.S and New Jersey.
  • Ability to serve as a representative of Prevention Links Inc, upholding agency's expectation of excellence and collaboration.
  • Maintain an understanding of all programs, grants, and projects offered by Prevention Links Inc.
  • Adhere to Prevention Links Inc administrative, reporting, and recordkeeping guidelines to support accurate documentation of activities.
  • Participate in all agency required training.
  • Ability to speak and write English.
  • Ability to walk, stand, and sit for long periods of time.
  • Ability to lift 25 pounds.
  • Valid New Jersey Driver's License and independent mode of transportation.
