Job Summary

The Senior Reentry Case Manager will provide comprehensive case management services, including assessment, goal setting, and linkage to stabilization services. You will work closely with our clients to identify barriers and strengths, and develop individualized case plans to meet their unique needs.

Responsibilities
  • Conduct thorough assessments of clients to identify their strengths, barriers, and goals
  • Develop and implement individualized case plans to meet client needs
  • Provide regular check-ins with clients to monitor progress and make adjustments to case plans as needed
  • Collaborate with community partners to provide wraparound services and support
  • Attend court hearings, school meetings, and other appointments as necessary to advocate for clients
  • Maintain accurate and up-to-date records of client progress and case plans
  • Participate in team meetings and contribute to the development of best practices and program improvements
Requirements
  • 2+ years of experience in case management, preferably with a focus on reentry services
  • Experience working with justice-involved youth and young adults, including those with complex needs
  • Strong assessment and goal-setting skills, with the ability to develop and implement individualized case plans
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, community partners, and colleagues
  • Ability to work independently and as part of a team, with a high level of flexibility and adaptability
  • Valid California driver's license and reliable vehicle for work-related travel
