Job Summary

We are seeking a highly skilled Occupational Therapist to join our team at Enhabit Home Health & Hospice. As an Occupational Therapist, you will play a vital role in helping patients with debilitating conditions improve their daily living skills and motor functions in the comfort of their own homes.

Key Responsibilities
  • Develop and implement individualized treatment plans to improve patients' daily living skills and motor functions.
  • Administer skilled care to clients requiring intermittent professional services and teach family members and other healthcare team members.
  • Collaborate with Branch Directors and other healthcare professionals to ensure seamless care delivery.
  • Conduct thorough assessments and evaluations to identify patients' needs and develop effective treatment plans.
  • Provide education and training to patients and their families on proper techniques and strategies for daily living and motor function improvement.
Requirements
  • Graduate of an approved school of Occupational Therapy and licensed as an Occupational Therapist in the state of practice.
  • Minimum of one year of experience in home health or skilled care setting, or equivalent experience with Regional Vice President and/or Regional Director of Therapy Services approval.
  • Medicare home health experience preferred.
