Responsibilities- Conduct comprehensive assessments of patients' needs, including OASIS assessments
- Develop and implement individualized treatment plans in consultation with physicians
- Provide skilled physical therapy services to patients in their homes
- Communicate effectively with patients, families, and healthcare team members
- Supervise Physical Therapist Assistants and ensure high-quality care delivery
- Graduate of an accredited Physical Therapy program
- Licensed to practice as a PT in the State of Florida
- Current BLS Certification
- At least one year of experience as a Physical Therapist
