We are seeking a skilled Physical Therapist to join our team at Revival Home Health & Hospice. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Responsibilities
  • Conduct comprehensive physical therapy evaluations and develop personalized treatment plans to meet the unique needs of each patient.
  • Collaborate with healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
  • Provide one-on-one therapy sessions in the patient's home, promoting independence and functional ability.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and seamless care transitions.
  • Stay up-to-date with industry developments and best practices through ongoing education and training.
Requirements
  • Graduate of a CAPTE-approved Physical Therapist Education program.
  • Current Physical Therapist licensure in the state and American Heart Association CPR certification.
  • At least two years of experience in adult physical therapy, with home health experience preferred.
  • Excellent observation, verbal, and written communication skills, problem-solving skills, and basic math skills.
