Telecare Corporation is seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for coordinating service needs and collaborating with other services and agencies to provide comprehensive support to our clients.

Key Responsibilities:

  • Complete assessments and determine the appropriate level of services for new members served.
  • Coordinate service needs and collaborate with other services and agencies.
  • Provide crisis intervention and support to clients in need.

Requirements:

  • Masters degree in Social Sciences.
  • One year of experience in providing crisis management and mental health services.
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
  • Willingness to use the company vehicle to drive clients to appointments and groups, etc.

  • Paid Time Off: For Full Time Employee it is 16.7 days in your first year.
  • Nine Paid Holidays & Shift differentials for hourly staff (6% for PM Shift, 10% for Overnight Shift). Weekend Shift differentials for hourly staff (5% for Weekend AM Shift, 11% for Weekend PM Shift, 15% for Weekend Overnight Shift).
