Job Summary:

Northern Rivers Family of Services is seeking a Licensed Practical Nurse to join our team. As a LPN, you will provide medical care and services to a youth and adolescent population with emotional, behavioral, and physical needs.

Responsibilities:

  • Coordinate medical care and services to meet the requirements of the regulating agency.
  • Report to a Registered Nurse and work onsite.
  • Complete nursing observations and report findings to the registered nurse.
  • Review lab and test results, reporting critical values to RN and providers.
  • Manage medications, including ordering and obtaining prior authorizations.
  • Communicate with medical director/providers regarding observations in client's health/behavior.
  • Provide client and family education regarding medications and treatments.
  • Collaborate with staff regarding concerns and behaviors.
  • Participate in Interdisciplinary Medication Review meetings.
  • Nursing documentation using EHR.
  • Update discharge planning needs.

Requirements and Education:

  • LPN Licensed in New York State.
  • 1 to 2 years of experience.
  • Experience working with youth and adolescent population desired.
