Responsibilities:
  • Provide comprehensive assessments and develop individualized plans of care in collaboration with the physician and other healthcare professionals.
  • Accurately and timely complete OASIS assessments, including medication reconciliation, and document patient visits per policy and payer requirements.
  • Initiate, develop, implement, and revise the plan of care as needed, in collaboration with the physician and other healthcare professionals.
  • Communicate effectively with the healthcare team, including but not limited to, patient care issues or needs, visit assignments, and schedule changes.
  • Supervise care provided by home health aides and licensed practical vocational nurses, and provide instruction as appropriate.
  • Participate in on-call rotation and weekend rotation as needed to meet patient needs.
  • Adhere to all LHC Senior Living service line processes, programs, and procedures, as well as the requirements, standards, policies, and procedures of the senior living communities.
Qualifications:
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
