Salary InformationThe estimated annual salary for this position is between $60,000 and $80,000, depending on qualifications and experience.
Job DescriptionJob SummaryWe are seeking a highly motivated and experienced Program Development Specialist to join our Bridges Program team in Montgomery County. The successful candidate will be responsible for assisting individuals in achieving their stated housing and personal goals, while maximizing independence and productivity through support and training.
Key Responsibilities- Develop detailed Bridges plans with each client, outlining steps necessary to obtain and maintain permanent housing;
- Monitor and coordinate delivery of necessary evaluations and assessments to identify individual needs;
- Plan and conduct individual housing action steps as indicated in the Bridges Plan;
- Support individuals' efforts to follow through on essential aspects of housing, such as obtaining ID, addressing legal issues, housing search, meeting with property managers, etc.;
- Make contact with new individuals within 24-48 hours of admission and meet with current individuals at a minimum monthly basis;
- Provide ongoing case management to individuals following placement;
- Refer individuals to needed services, including health, mental health, education, and vocational training;
- Serve as an advocate or liaison to other organizations when needed;
- Ensure required elements of ECR/EHR are present, current, and in accordance with agency certification and documentation standards;
- Bachelor's degree in social work, human services, behavioral health, or a related field;
- Two years of experience working with families and/or children in the child welfare system preferred;
- Initiative and ability to work independently;
- A willingness and comfort level in providing community-based services to a diverse population of clients;
- Excellent problem-solving skills and the ability to remain calm and professional in crisis situations;
- Must be 21 years of age; have a valid driver's license and current automobile insurance;
- Able to pass a criminal background check and drug screen;
