Job Title: LPN Home Health

Job Summary:

Maxim Healthcare Services is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to provide high-quality home health care services to our patients. As an LPN Home Health, you will be responsible for delivering patient-centered care, collaborating with healthcare teams, and ensuring the highest level of patient satisfaction.

Key Responsibilities:
  • Deliver skilled nursing care and services to patients in their homes, as prescribed by physicians and in accordance with state regulations.
  • Conduct thorough assessments of patients' physical and psycho-social conditions, identifying areas for improvement and developing individualized care plans.
  • Implement and evaluate the effectiveness of interventions, treatments, and medications, ensuring patient safety and well-being.
  • Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure comprehensive patient care.
  • Document patient interactions, assessments, and care plans accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
  • Participate in case conferences, team meetings, and education in-services to stay up-to-date on best practices and industry developments.
Requirements:
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Current licensure as a Licensed Practical Nurse in the state of Pennsylvania.
  • One year of prior professional nursing experience, preferably in home health care.
  • Willingness to participate in skills advancement training or novice nurse program, if applicable.
  • Current CPR certification, as applicable, for direct patient care.
  • PPD or chest x-ray, if applicable.
  • Successful completion of Maxim Orientation and Compliance Training, and all other assigned trainings, as applicable.
  • Successful completion of Maxim's Competency Program.
  • Proficiency in the English language.
  • Current Health Certificate, per facility contract or state regulation.
  • Must meet all federal, state, local, and contract requirements.
